Narrative

An employee was loosening a hex nut on the coating head of the machine to replace it. The employee dropped the adjustable wrench he was using and it fell onto the rollers of the machine. Upon reaching for the wrench, his finger was pulled into the machine and pinched between the rollers and a strip of steel. The employee sustained a partial amputation to the right index finger.
