Narrative

An employee was using a 20-foot section of galvanized pipe to unclog a section of dust pipe in a baghouse. A tip of the galvanized pipe broke through the clogged section, contacted the relay fan, and rotated, pulling away from the employee's hand and striking the right side of his head. He suffered a laceration and a brain injury.
