Narrative

Employees were removing the lid of an access hatch from the top of a clearwell tank. The hatch lid (weighing 75-100 lbs.) slipped and landed on an employee's left middle finger, pinning it against the concrete lip of the hatch. The employee's finger was partially amputated.
