Narrative

A temporary employee was hosing and washing down a warehouse floor with water/soap and sanitizer. He tripped on a metal strip attached to the docking floor plate, slipped on the mix of water and soap, fell on his back, and slid off the dock. He suffered a bleeding wound to the back of his head, as well as a back injury. He was hospitalized.
