Narrative

An employee was in the back of a vehicle, moving skids of freight to access a shipment for unloading. As he pulled a skid, its plastic strapping/banding broke, throwing him off balance. He fell out of the back of the truck, landed on his right shoulder and back, and suffered injuries to both.
