Narrative

An employee was on a roof, securing an anchor for his fall arrest system. When he stepped on a 6-inch-wide roof panel near the roof ridge, it did not hold his weight. He fell to the ground 36 feet below and suffered a broken right wrist, as well as injury to the left wrist.
