Narrative

An employee was calibrating a mixer. Two chemicals (a furan no-bake resin and an acid catalyst) were mixed in a bucket, reacted, and splashed onto the employee's face and down his arms. The employee suffered chemical burns to the face and arms. Personal protective equipment was not being worn at the time.
