Narrative

The employee was installing hat channels on the roof of the carport. He had placed one set of the hat channels and climbed on top to start placing the second set of hat channels when the hat channel previously installed broke off. The employee fell onto the concrete. The employee sustained a left wrist fracture and a skull fracture.
