An employee was assisting to lift and set a stacker machine that had fallen onto its side. The employees were lifting the stacker with a come-a-long winch attachment affixed to the forklift. When the machine was attached to the rigging equipment, the machine shifted, causing the stacker to move. The employee's right middle finger was then caught between the stacker and the wooden counter balance beneath the stacker, resulting in an amputation of the right middle finger.
