Narrative

On December 8, 2022, at 8:30 p.m., an employee and a co-worker were switching out trucks when the fifth wheel of the truck did not unhook from its trailer. The employee reached in to switch the lever to unhook the fifth wheel. The truck moved forward and drove over the employee's right foot, resulting in a foot injury that required hospitalization.
