Narrative

An employee was purging a fuel system on a vibratory plow to repair the fuel system. The motor on the vibratory plow surged, causing the revolutions per minute to significantly increase. The employee went to manually shut off the fuel and his hand slipped and contacted the fan. His right little finger was amputated.
