Narrative

An employee was delivering a load of concrete to a customer with a truck. The employee walked around to the admix tank on the truck to add the concrete accelerator to the load of concrete when the PVC tank end cap blew off and struck the employee, resulting in blunt force trauma to the neck and torso.
