Narrative

An employee was moving special tooling back into a shipping stand when the locking pin latched incorrectly, allowing the counterbalance weights to slide to the end of the tooling. The weight slid past the safety stop, pinching the employee's right middle and ring fingers. The employee sustained two fingertip amputations.
