Narrative

An employee was working on a malfunctioning pneumatic arm that grabbed tire tread and pulled it onto a conveyor belt. While the employee was investigating the malfunction, the employee's right little finger was caught in a pinch point on the arm. The employee suffered an amputation to the finger.
