A 48-inch steel pipe was being installed under railroad tracks. The pipe encountered an old timber bridge. The employee entered the pipe with a gas-powered chainsaw to cut out the obstruction. Afterwards, the employee became dizzy and fell. He was hospitalized for carbon monoxide exposure.
