Narrative

An employee was performing maintenance on a scissor lift by aligning a bolt on a part suspended by hoist; the base part of the lift was on the worktable. The part would not align properly and the employee went to see what was caught. The material freed and pinched their left index finger between the two parts, causing an amputation of the fingertip.
