At about 1:30 p.m. on March 7, 2023, an employee was clearing a material jam in metal-bending equipment. The equipment activated while the employee's right hand was inside it, resulting in an amputation to the index finger from the nail bed to the first knuckle. The equipment's side guard had been removed at the time of the incident.
