Narrative

An employee was moving a 1,100-pound staircase that was rigged with an electric hoist with a cable and hook with a safety clasp. The safety clasp failed, causing the staircase to fall and strike the employee in the head and torso. The employee was knocked to the ground and sustained head injuries, fractured ribs, and internal injuries.
