Narrative

An employee was driving an electric pallet jack toward the receiving department when the vehicle drove over a piece of cardboard. The vehicle then struck a pallet of wine and the employee's foot was wedged between the pallet and the electric pallet jack. The employee sustained a partial right heel avulsion.
