Narrative

An employee was blowing out a surge bin on a blow mold machine for a material changeover. When he went to clear material out of the top rotary air lock area, it opened and closed on his middle three fingers. The right ring, middle, and index fingers were amputated. The ring and middle fingers were surgically reattached.
