Narrative

An employee was performing maintenance on a cold header machine in the MVH. He was lining up a pin hole to put a one-inch round pin between the arm of the cold header machine and a piece of metal. The arm slipped out of his hand and dropped causing a partial amputation to the employee's left index finger.
