Narrative

Two employees were troubleshooting a problem with an envelope folding machine. The injured employee stopped the machine and began inspecting the belt. As he manually gauged the tension, his finger was pinched between the belt and pulley, resulting in a de-gloving injury and fracture of the right index finger.
