Narrative

An employee was installing a rake arm to a drive shaft of a clarifier and tightening bolts on the clarifier rake arm. The shaft separated from the expansion joint, causing the unit to drop approximately 3 feet and strike the employee on the top of their right foot. They sustained a fracture.
