Narrative

An employee was on the platform of a pump truck. He shook the hose to return the residual concrete in the hose to the hopper and tie off the hose. Air released causing the hose to swing and knock the employee off balance. He fell approximately 4 feet off the pump truck and struck his head and shoulder on the ground. The employee sustained fractures to the clavicle and skull as well as brain hemorrhaging and a concussion.
