On 9/09/2023, at 12:20 AM, an employee was operating a machine that routes pieces of wood. She inserted a piece of wood and her right index finger became caught between the wood and the hold-down of the machine. The employee sustained an open fracture to the distal phalanx and amputation of the fingertip.
