Narrative

An employee was replacing hinge bolts and hinge pin bearings (carrots). His right index finger was in the top eyelet of the hinge to manipulate the carrot when the door shifted. The employee's right index finger was caught between the hinge and door, resulting in amputation at the medial/distal phalange.
