Narrative

An employee was assisting with the fabrication of a circular metal flange 24 inches in diameter. As the employee was turning the blind, it rotated with the employee's finger in the bolt hole. His left middle finger was then caught between the bolt hole and the table it was resting on, resulting in amputation.
