Narrative

An employee was assembling the arm of a hay merger (piece of equipment that picks up hay and places it onto a conveyor). As he was mounting the bracket hole to install the pin, the cylinder shifted causing it to extend further and catch the tip of the employee's finger between the mounting hole and the mounting plate. The employee sustained a left index fingertip amputation.
