Narrative

Employee 1 was operating a telehandler that was carrying a crate of glass weighing in excess of 3,000 pounds. The crate began to slip. Employee 2 was nearby and reached to stop the crate from falling, but the crate fell and amputated the fourth finger of their left hand.
