Narrative

On October 14, 2023, an employee was removing a rubber hose that had been wound on a removal tool that unplugs it from a vacuum conveyor. The hose wraps on a cone until it over-torques and shuts off. The hose had stored residual energy and unwound, pinching and amputating the employee's right middle fingertip.
