Narrative

On October 19, 2023, two employees were trying to loosen a stuck lug nut on a tire rim. The injured employee was holding onto the wrench while the other employee hit it with a hammer. When the wrench was hit, it came loose and struck the injured employee, causing a broken nose requiring amputation.
