Narrative

An employee was using a driver mounted on a flex arm to torque nuts onto studs on wheel assemblies. The employee's gloved left hand slipped off the non-rotating speed collar and became caught on the rotating driver shaft. The employee's distal phalanx on their left thumb was amputated.
