Narrative

Employees were installing a retainer plate weighing approximately 95 pounds on a front corner group on a dump truck. A prybar was used to align the retainer plate. The prybar slipped and the retainer plate fell crushing an employee's right index finger against the floor, resulting in a partial amputation.
