Narrative

An employee was walking on a newly installed 12-inch water line so they could insert a plastic night plug into the 45-degree fitting that was bolted onto the end of the pipe. The employee slipped off the side of the pipe and fell approximately 2.5 feet to the ground. As the employee fell, their left leg struck one of the bolts on the fitting and was lacerated. Additionally, the left fibula was broken.
