Narrative

An employee was straightening material on site to consolidate and cover it. As he was pivoting while moving a bundle of 6-inch pipes, he tripped on dunnage and fell. As he fell, he put his hands out to catch himself. The pipes shifted and contacted his left middle finger, resulting in an amputation down to the first knuckle.
