Narrative

On December 1, 2023, an employee was performing preparation work for installation of an electrical control panel on the deck mezzanine (metal grate platform) of a commercial site. The employee tripped on the leg of an A-frame and fell through a floor opening. He landed on a toolbox 8 feet below, then fell another 7 feet onto concrete pavement. The employee sustained severe injuries and was hospitalized.
