Narrative

The employee was using a manual style pallet jack to move a pallet full of glass panels. The employee was attempting to get the forks situated under the pallet. The pallet became uneven causing the glass panels to fall off the pallet. The employee attempted to stop the glass from falling and their arm was lacerated by the glass, resulting in hospitalization.
