Narrative

An employee was operating a reach truck down an aisle in a warehouse. As the reach truck turned to pick a pallet, the vehicle struck a pallet jack, causing the employee's foot to become caught between the pallet jack and a rack. The employee suffered blunt force trauma to the left foot.
