An employee was in a 13-foot trench busting concrete with a sledgehammer and a drill to fit a pipe in a junction box. The trench collapsed and dirt hit the back of his legs, causing him to hit his face on a piece of rebar and concrete. The employee suffered a puncture wound and fractures to his face below the right eye.
