Narrative

An employee was pushing a 45-pound sleeve off a cylinder on a printing machine after it had become stuck. The actuating arm activated and pushed the sleeve, crushing his finger and resulting in an amputation to his right little fingertip. The machine was not locked out at the time of the incident.
