A storage silo imploded due to catastrophic failure and it collapsed, releasing 3,500-3,700 tons of fly ash which struck a 20- x 40-foot shipping container with maintenance supplies. When the silo collapsed, the injured employee was working near the container and was knocked to the ground. The shipping container was pushed onto their right arm, catching it between the container and the ground. The arm sustained an injury that resulted in surgical amputation.
