Narrative

An employee had unloaded a skid from a semi-truck trailer and put the skid on the ground. The employee went to move the forks on the forklift to a wider position. He was pulling the left fork outward to move it further out on the mast when the fork came off the mast. The fork fell and fractured his right foot.
