Narrative
An employee was filling a rocking dumpster with refractory rubble and disposing of it into a bigger dumpster. The rocking dumpster was on the forks of a forklift. His left hand was on the bottom of the dumpster as he went to pour the rubble into a bigger dumpster below. When the rocking dumpster tipped over his left index, middle, and ring fingers were crushed between the dumpster and the forklift forks.
