Narrative

An employee was filling concrete forms. They were lowering the concrete pump hose into the forming when an air bubble caused the hose to kick back and strike the employee's back causing a fractured L5 vertebra. The employee then landed on formwork and lost two teeth. The employee was hospitalized.
