Narrative

An employee was extracting a sample out of a Shelby tube using a hydraulic cylinder system. The Shelby tube got stuck on the hydraulic cylinder due to thick clay buildup. While pulling the Shelby tube apart from the hydraulic cylinder, the valve for the hydraulic system failed and the hydraulic cylinder engaged amputating his right index finger at the first joint.
