On June 29, 2024, the injured employee was climbing a 10-foot fixed ladder on a mixing tank to check the process. A forklift approached the area carrying a hopper to add to the tank. The injured employee became concerned about the approaching forklift and jumped from the ladder to the concrete floor, sustaining four broken ribs on their right side.
