Two employees were rotating a steel plate weighing 17,558 pounds that was suspended from a crane. The safety chain spun around the crane and broke which unhooked the electrical cord and deenergized the magnet. The plate fell off the crane and onto the employee's foot causing a foot amputation. The employee then fell backward and broke his wrist.
