Narrative

The injured employee was repairing a broken pin on the planer outfeed roll cylinder. A hoist was being used, and the employee was aligning a piece of the equipment when the cylinder moved and the employee's index finger was pinched between the clevis hole and the cylinder hole, causing a fingertip amputation.
