Narrative

An employee was on a ladder troubleshooting an emergency light that was out in the ceiling. He removed a wire nut and a live wire sprung back, hitting his finger and shocking him. The employee jumped and fell off the ladder onto his back on the floor. He lost consciousness and sustained a head laceration and fractured vertebrae. He was wearing insulated boots and using insulated equipment.
