The injured employee was operating a riding pallet jack. He was entering an aisle through an overhead door while another pallet jack operator was exiting through the aisle s overhead door. As the employees went to pass each other, the injured employee's right little toe struck a yellow bollard that was guarding the pedestrian door. The toenail and the upper layer of skin were torn, and he was hospitalized with a fracture.
